We believe that unhurried and comprehensive evaluations offer you the best chance of healing and getting back to your life. Please read through all of the below to best prepare for your appointment with us.

What is a Neurology Specialist?

As a Specialist Neurology Practice (rather than a Primary), we care for conditions and symptoms specifically affecting one's nervous system. This means we will need a referral from your existing Provider to see you.

My appointment is scheduled, what do I do next?

All you need to do is complete your New Patient Paperwork and help us retrieve any neurology records you may have. If you need to cancel or reschedule, please notify us at least 24 hours before your appointment to allow us to serve other patients during this time.



What paperwork do I need to complete?

Please complete your New Patient Paperwork and return to us via mail, in-person or on our patient portal at least one week prior to your appointment. You can download it here:

What medical records do you need?

If you have seen a Neurologist before, please send us a copy of your Neurology records before your appointment so that our team can review them for your treatment. You can retrieve these from your existing Provider. If we require other medical records, we will let you know or consult your Primary Care.

When do I need to arrive?

Please arrive 20 minutes before your appointment and bring your current photo ID and health insurance card(s) with you.

What if my symptoms get worse?

If symptoms worsen before your appointment, please notify your Primary Care Provider for them to determine the urgency of your appointment with us.

NEW PATIENT PAPERWORK


Our goal is to provide our patients with the most efficient and comprehensive care at the time of your visit. In order to achieve this, we require all previous records to be received from your doctors, prior to the scheduling of your appointment. Once your appointment is scheduled, we will be reaching out and arranging for you to complete the new patient paperwork prior to your visit. We will provide you with a link to sign up for our patient portal which you can then access on this website. If you are unable to access the portal, please reach out to our office staff at 530-999-2533 or info@catalystneuromedical .com and they will assist you in signing up or mailing out the appropriate forms to you.


In an effort to maximize the time you spend with your Provider:


· New patients should plan to arrive at least 20 minutes prior to your appointment time to allow time for completion of necessary paperwork. 

·  Please bring a current photo ID and health insurance card(s) to each visit.

CANCELLATION AND

NO-SHOW POLICY



To provide excellent medical care to all patients, we require at least 24 hours notice of cancellation of an appointment. A $75 no-show fee will be charged if at least 24 hours notice is not provided before cancellation. (We realize that emergencies do occur, and you may not be able to notify us. We are happy to discuss this on an individual basis).






BILLING AND INSURANCE POLICIES


The following information covers common billing and insurance questions. If your situation requires a more personalized response, please contact our billing office at (530) 999-2533 option 4 during business hours.


We understand health insurance can be complicated, even for those who work with it daily. To ensure you receive the greatest benefit from your health insurance carrier, we encourage you to familiarize yourself with your policy and benefits. Our helpful and experienced staff are available to assist you should you have further questions.



Financial Policies


Payment of the patient responsibility portion of your visit is due at the time of service including all applicable co-payments, deductibles and coinsurance amounts. You will receive notification of any services not covered by your health plan, along with a bill for the unpaid balance. If you are without current health insurance, cash payment is expected at the time of service.


Our office anticipates payment within 30 days of billing. Accounts are considered delinquent after 90 days. We accept cash, checks, most major credit cards, and money orders. Please direct any billing or insurance questions to our billing team at 530-999-2533 option 3.



Insurance Policy


Catalyst Neuromedical Center is a Medicare Part B provider. We also accept self-pay and out of network patients and would be glad to assist you in arranging for payment of services.


After services are provided, our office will bill your health insurance and we will be glad to file claims to your insurance company on your behalf. We will assist you in making sure that your insurance pays us timely and appropriately for services we have provided to you. However, please know that ultimately you are responsible for ensuring payment is made to us by your insurance.
